CareSouth’s Diabetes Disease management program is designed to assist the physician in meeting the unique challenges of this disease and provide improved outcomes in managing the patient’s general health.
Our program is based upon National Standards of the ADA, Agency for Health Care Policy and Research, and the National Institutes of Health.

- Hyper/Hypoglycemic Reactions
- Nutrition and Diet
- Prevention of Complications
Our Diabetes Disease Management program is designed to deliver quality home care services through staff trained specifically in diabetic management and assessment.
CareSouth measures program effectiveness through outcome data reporting as well as patient and physician satisfaction.
