Congestive Heart Failure, CHF, is listed as the primary or secondary diagnosis for one out of five discharged patients age 65 and older. CHF occurs when the heart muscle does not pump enough blood to meet the needs of the body. The main causes of weak heart muscle are:

  • Blockage of the coronary arteries by cholesterol plaques
  • Malfunction of heart valves
  • Virus infections of the heart muscle

Managing cardiac disease effectively can reduce symptoms, improve quality of life, and improve the chances of a patient’s return to independence.

Patients experiencing any of the following signs/symptoms may benefit from the CareSouth Congestive Heart Failure disease management program:

  • Frequent dry, hacking cough
  • Fatigue, weakness, dizziness or fainting
  • Swollen feet, ankles, and legs
