ADMISSION CRITERIA
Diagnostic Skilled Care Indicators

  • Recent discharge from a psychiatric hospital or unit
  • Recent psychiatric crisis or suicidal ideation
  • Changed or new medication regime
  • Sudden or severe change in behavior
  • Sleep disturbances
  • Axis-I Diagnoses such as:

        Schizophrenia
        Bipolar/Unipolar Disorder
        Major Depression
        Anxiety Disorder
        Panic Attacks
        Post Traumatic Response
        Chronic Pain
        Body Image Disturbances
        Anorexia
        Bulimia
        Amputations
        Surgeries

Requirements for Care

MD Order specific to Mental Health Measurable Rehab potential

Normal inability to leave home due to psychiatric illness, or it requires a considerable taxing effort and patient has one or more of these symptoms:

  • Severely depressed
  • Impaired judgement/thought process
  • Excitable and impulsive
  • Severe Anxiety or panic attacks
  • Delusions
  • Hallucination
  • Paranoia
  • Suicidal ideation
  • Emotionally labile
  • Intense OCD behavior
  • Poor reality orientation
  • Aggressive
  • Hostile
  • Angry
  • Impaired socialization skills and basic self care
