CareSouth Home Health offers a wide range of services to cover most patient needs for medical care and rehabilitation at home. Our clinical team works within the physician’s desired protocol, following up with frequent reports on the patient’s treatment and recovery status. This at-home care gives patients a healing comfort and security that only the familiar sights and sounds of home can provide.

The focus and drive at CareSouth is to insure our patients a smooth, resourceful, and timely progression through the healing process. Skilled and compassionate care is delivered by a team of carefully selected clinicians under the guidance of the physician’s plan of care.
Skilled Nurse: develops and carries out the specific plan of care for each patient. Services include skilled care and evaluation of patient needs, education of patients and caregivers, and coordination of care with the patient’s health care team.

Physical Therapist: provides evaluation and training in mobility, transfers, gait, pain, strength and therapeutic exercises, and instruction on equipment and home safety.
Occupational Therapist: evaluates and trains patients in daily living activities of feeding, bathing, and dressing. The therapist leads the patient in an exercise regimen to build strength, increase range of motion, and improve coordination. The Occupational Therapist also evaluates patients for adaptive equipment and splints.
Speech Therapist: evaluates and trains patients who have speech, language, voice, swallowing and cognitive disorders.
Medical Social Worker: provides short-term counseling services and makes referrals to appropriate community resource for assistance with living arrangements, finances, and other long-range planning.
Certified Home Health Aide: delivers services under the supervision of a registered nurse and/or licensed therapist. Provides in-home personal care duties such as bathing, assistance with meals, and activities of daily living.
