Admission Coordination Program
CareSouth developed the Admission Coordination Program (ACP) to provide more customer service and excellent care to our patients and for our referral sources. Services provided by ACP include:
- Visits to patients' homes, hospitals, Assisted Living Facilities, Skilled Nursing Facilities and Nursing Homes to coordinate care prior to admission.
- Communication with our patients and their families about what to expect during the episode of care.
- Ordering and securing equipment or supplies.
- Evaluation of patients' homes to determine whether the environment will support home care services.
- Identification of individuals willing and able to serve as primary caregivers.
