CareSouth Connect - Telehealth
What It Is
If you are home bound and have had frequent hospitalizations or trips to the emergency room or medical clinics, you may be a candidate for CareSouth Connect.
CareSouth Connect, also known as telehealth or remote vital sign monitoring, uses a transmitter that connects to your phone wall jack to carry messages from wireless monitoring devices to a remote monitoring system. Examples of these devices are a PT/INR Monitor, a Blood Pressure Device or a Glucometer.
How It Works
Whenever you take your vital signs, that information is transmitted to a monitoring center. There your information is reviewed and reported to your home healthcare provider for evaluation. If necessary, your doctor is contacted.
Studies have shown that remote vital sign monitoring has helped reduce additional hospital stays by proactively evaluating trends that may signal a recurring health problem.
