Stand Strong - Balance Program
CareSouth's Stand Strong program was developed to help Assisted Living Facilities and Nursing Services Facilities in the prevention of falls. Included in the program are Fall Prevention with Community Balance Screenings, Educational In-Services, Family Meeting Facilitator and Coordination of Care.
Community Balance Screenings include
- Coordination of community invitation mailings to facility events
- Provide a Licensed Therapist for screenings
- Provide names to facility wellness nurse of low scoring residents
- Schedule regular health screenings
- Education In-Services include
- Provide facilities with a skilled clinician to speak at scheduled staff, family or resident council meetings about health and wellness.
Family Meeting Facilitator include
- Make available a nurse or therapist to speak with patients and families about home health services.
- Communicate with families and staff about any on-going exercise plan for maintaining residents' achieved level of function.
Coordination of Care include
- Provide documentation to facility wellness nurse supporting the need for extended services once a need is identified.
- Link facility and residents to the most appropriate community resource.
